PHYSICAL EXAMINATIONS
Parents are reminded that all students entering school for the first time (Kindergarten or First Grade) and all transfer students from out of the State of Nebraska, regardless of grade, are required to have a Physical Examination.
The Physical Form (pdf) is available for parents/guardians to download. The State of Nebraska does also allow for parents to have the option to waive the required physical exam by signing the Physical Exam Waiver (pdf).
